FT position available (40 hours) - primarily day shift

Responsibilities

Exciting opportunity available for an Oncology Nurse to provide care coordination in the clinical management of survivors as a part of the Cancer Survivorship Program at the Wilmot Cancer Institute (WCI). The cancer survivorship program is dedicated to improving survivors’ quality of life, providing education regarding treatment received, late treatment effects and their management, as well as cancer prevention and lifestyle management. The WCI utilizes a multidisciplinary approach to survivorship care based on needs assessment and resource allocation. The survivorship coordinator will work closely with the Survivorship APP in the coordination of patient care and program development. Specifics of the role include: The Survivorship Program Coordinator will perform initial introductory phone call and ensure completion of the survivorship needs assessment prior to the initial encounter and at specified intervals, assist in providing survivorship ongoing education and support, perform quality/safety/research initiatives to meet the program’s goals and metrics, participate in WCI survivorship committees and community programs, coordinate and develop survivorship care plans, and navigate survivorship transitions and continuity of care for cancer survivors and their caregivers in collaboration with their medical team. In collaboration with the program directors and the Survivorship APP, the survivorship coordinator will develop and implement strategies to foster growth of the survivorship program.

Qualifications

BSN required, certification preferred. A minimum of 3-5 years’ experience in Oncology nursing practice is preferred. Ability to work independently and self-directed is necessary.
