Responsibilities

The RN care manager will be responsible for a caseload of patients and will work as part of a multidisciplinary treatment team in the provision of psychiatric care to individual patients, families/support system in accordance with a multidisciplinary/patient centered treatment plan. The RN care manager will monitor and assess patient care needs and provide additional support, assessment, and interventions regularly based on the clinic standards of care. The RN care manager will be required to monitor for resurgence or exacerbation of symptoms, providing education around medication adherence, and will seek to find solutions consistent with clinic and community based resources. The RN care manager is responsible for outreach and engagement efforts to patients who are not engaged in care with the psychopharmacology provider. The RN care manager will monitor daily hospitalization list to coordinate with the inpatient team, psychopharmacology provider, and all other members of the patient’s care team to coordinate services to support outpatient care upon discharge. Must possess strong communication skills. Minimum 2 years of Psychiatric Mental Health experience required, outpatient experience preferred.

Qualifications

Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 6-12 months of RN experience. This position assumes the charge nurse role, actively participates in unit/service activities, supports co-workers, and assesses unit needs.
