GENERAL PURPOSE:

Responsible for identifying, establishing, and maintaining relationships with affiliated referring clinics / hospitals, staff, and communities throughout our region. Participates in case review and standardizes referring guidelines for all centers. Creates and manages guidelines for outside centers to use for early referral of advanced heart failure patients. Promotes adherence to nationally accepted standards of care from ISHLT, AHA, HFSA, and ACC.

RESPONSIBILITIES:

  • Collaborates with Advanced Heart Failure Program Leadership regarding outreach opportunities with referring centers and clinics.

  • Collaborates with the Advanced Heart Failure Program Educator to educate the greater referral community on opportunities to optimize patient care.

  • Collaborates with the VAD and Heart Transplant Coordinators to assist with outreach when patients return to outlying communities after VAD implant or heart transplant.

  • Designs and implements referral guidelines for outpatient and inpatient referring centers, goal for early and appropriate referral of Advanced Heart Failure patients.

  • Develops and implements a process for patient follow-up (report cards) with referring centers regarding patient outcomes and feedback for future patient referrals.

  • Develops and implements a quality improvement process for referrals from outside hospitals.

  • Coordinates outreach educational sessions with referring centers and clinics and the Advanced Heart Failure team.

  • Assists with coordination of division involvement in meetings, forums, training sessions and conferences, as needed.

  • Works closely with the Advanced Heart Failure leadership team to identify knowledge gaps in our community and appropriately target outreach activities in these specific areas.

  • Delivers presentations to interested groups.

  • Participates in Advanced Heart Failure research and quality improvement activities within the program.

  • Reviews INTERMACS and other national reporting data and implements outreach activities in areas where our greater community could be served by AHF advanced therapies.

QUALIFICATIONS:

Required:

  • Individual must have a bachelor’s degree, masters preferred.

  • 2 years clinical experience in Advanced Heart Failure including experience with mechanical circulatory support.

  • Demonstrate knowledge in the practice of Advanced Heart Failure care and early referral for advanced therapies.

  • or equivalent combination of education and experience

Preferred:

  • Self-directed and resourceful in problem solving, critical thinking

  • Effectively handle multiple priorities

  • Experience in developing, implementing, and evaluating the effectiveness of community benefit programs

  • Strong interpersonal, presentation, organizational, planning and communication skills, including written communication

  • Knowledge of communication theory and public relation techniques

  • Basic graphic design principles

  • Basic desktop publishing techniques

  • Basic knowledge of Microsoft Office

  • Experience with data reporting / Excel report writing

  • Effective time management skills and attention to detail.

Pay Range

Pay Range: $68,765 - $103,147 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
