Responsibilities

General Description:

The social worker/social worker assistant, in collaboration with the interdisciplinary care team, provides social work clinical services, including discharge planning, to individuals and families of all ages who come to the Emergency Department for care. TAR social work staff provide coverage in the ED for all shifts: evenings, weekends, holidays, days and overnights.

The social worker also provides consultation to staff of other hospital units regarding psychosocial issues and community resources if the clinical backup social work staff are unavailable.

Specific Responsibilities:

Clinical:

Performs risk screening and evaluation of ED patients with age greater than or equal to 80 years, complaints suggestive of abuse or neglect, alcohol or substance abuse or domestic violence.

Performs psychosocial assessments of patients identified through risk evaluation or refen-al.

Collaborates with ED physician, nursing and Community Health Nursing staff in developing an interdisciplinary care and/or discharge plan for the patient.

Provides social work interventions to patients and their families including emotional support in crisis situations, information about and referral to community resources, facilitation of patient transfer to other care facilities and other services relevant to the patient's discharge from the ED.

Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence.

Notifies the inpatient social workers or CHN's of admitted patients when the ED social worker has gathered relevant psychosocial information.

Notifies the Social Work Administrator on call of potential non­ acute inpatient admissions and collaborates with the Manager in activities to divert the admission.

Provides consultation to other ED staff about psychosocial issues and resources for individual ED patients.

Provides consultation upon request to other hospital units regarding psychosocial questions and community resources if clinical back-up social work staff are unavailable. In very selected cases and only if not needed currently by ED patients or staff, may provide direct social work services to patients of other units.

Develops and maintains knowledge of hospital and community resources relevant to the needs of ED patients and their families.

Documents all activities in the patient's ED chart, or, in the case of other unit patients receiving direct social work service, in the patient's hospital medical record, in accordance with the Social Work Division policy and procedure.

Notifies CPS consultant of all possible cases of abuse or neglect. Collaborates with CPS to ensure safe & appropriate discharges of Pediatric patients with complaints suggestive of abuse or neglect.

Provides coverage services that are requested during the absence of co-workers.

Performs other duties as assigned.

Administrative:

Participates in ED social work staff meetings.

Participates in social work supervision/ consultation about his/her work in accordance with plan developed with supervisor.

Paiiicipates in required activities of the Social Work Division and Hospital including, but not limited to, timely completion of statistical reports/forms, time sheets and other Hospital or Division forms; mandatory inservice training programs and University Health Service reviews.

Participates in social work tasks and committees as assigned to develop policies, procedures and information required for the social work program in the Emergency Department.

Participates in assigned committees of the Social Work Division

Professional Accountability, Supervision and Evaluation:

Supervision/Consultation:

The Senior Social Worker and the Manager of Adult Social Work Services supervise the social worker's/social work assistant's professional performance.

Accountability :

The social worker/social work assistant is accountable administratively and clinically to the Senior Social Worker and the Manager of Adult Social Work Services for the provision of social work services in accordance with Social Work policies, procedures and standards of conduct. The social worker's/social work assistant's job performance must conform to the expectations set forth in this position description.

The social worker/social work assistant is accountable to the Director of the Emergency Department for providing social work services in accordance with policies and procedures established by the Emergency Department.

The social worker is expected to demonstrate the Strong Commitment - "I Care" values.

Evaluation:

Social Work evaluations are conducted by the Senior Social Worker and the Manager of Adult Social Work Services at the end of the first six months of employment and at least annually thereafter. Salaiy and promotional recommendations are made by the Senior Social Worker and the Manager of Adult Social Work Services in accordance with guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

Position Qualification:

The social worker must have an MSW from an accredited social work graduate prograin and must obtain New York State licensure within 18 months of employment.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
