GENERAL DESCRIPTION :
The social worker assigned to the Strong Ties Clinic is responsible for providing psychosocial assessment and social work treatment to individuals ages 18 and older who are severely and persistently mentally ill. This is provided through the social workers participation on an interdisciplinary team designed to provide comprehensive community based psychiatric care. The social worker functions as primary therapist for assigned patients and provides direct clinical service including individual and family therapy, group therapy, crisis intervention, and community outreach. The social worker may collaborate in policy and program development, operations managing, and plan program activities for the Strong Ties Clinic.
SPECIFIC RESPONSIBILITIES :
Attends Social Work Division, Social Work in Psychiatry and Program staff meetings.
Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice and patient care.
Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities.
Participates in activities of the Social Work Division including, but not limited to timely completion of statistical reports/forms, time sheets and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.
Subscribes to the Departmental of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
Participates in available cultural competency events and training appropriate to job duties
Participates as a member of an interdisciplinary treatment team involving consultation, psychosocial diagnosis, treatment planning, periodic care review.
Provide direct clinical services to assigned patients, which may involve individual, family, and/or group therapy, and crisis intervention. Patient care also involves referral to or liaison to other health and social agencies in the community.
Develops and lead groups for patients and family members which focus on issues and needs of the severely and persistently mentally ill population.
Documents treatment in patient’s chart in compliance with Division, Department, and Hospital Policy.
May provide backup social work services as requested to other units/programs during the absence of certain co-workers.
Based on experience and as determined by the social work supervisor, the social worker may supervise social work graduate students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.
Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.
PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION :
The social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).
The social worker is accountable clinically and administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The senior social worker's job performance must conform to the expectations set forth in the position description.
The social worker is accountable to the Director of Strong Ties Clinic for providing professional social work services in accordance with the policies and standards established by the Service and the Department of Psychiatry.
Social work evaluations are conducted annually by the social worker's supervisor with commentary from Strong Ties Director and/or designee. Annual salary and promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and professional promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.
POSITION QUALIFICATIONS :
The social worker who occupies this position must have a MSW from an accredited social work graduate program and a New York State license within eighteen months of employment. This position also requires experience in a mental health setting, preferably with the severely and persistently mentally ill population.
POSITION DESCRIPTION MAINTENANCE :
This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Clinic Director, and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of the Clinic. Any position modification is authorized by the Social Work Division Clinical Manager for Psychiatry.
