  • General Responsibilities

The Eastman Institute for Oral Health (EIOH) Social Worker is responsible for providing clinical services to patients to assist EIOH professional (and support) staff in the delivery of comprehensive care to EIOH patients. The EIOH social worker is responsible for liaison with community agencies and other relevant providers, which impact patients at EIOH. The EIOH Social Worker provides direct clinical services to identified EIOH patients and their families to increase access to and improve utilization of dental services, on behalf of low-income and vulnerable individuals/families. The EIOH Social Worker also participates in interdisciplinary collaboration with EIOH staff, with respect to program development, grant writing, research, and teaching, including the orientation and training of new staff and residents.

Specific Responsibilities


  • The EIOH Social Worker provides risk screening and psychosocial assessment and of referred EIOH patients. The Social Worker provides brief supportive counseling as appropriate to address identified psychosocial issues impeding access/adherence to dental care. The Social Worker consults with EIOH personnel regarding care planning. In addition, the Social Worker evaluates and makes recommendations regarding system-related issues, as well as. refers patient and family to appropriate community resources to enhance continuity of care. The EIOH Social Worker assesses and manages risk situations such as child abuse/neglect, domestic violence and suicidal ideation.

  • The Social Worker confers with physicians and other inter-disciplinary staff relative to overall planning for patient care and collaborates with hospital and community stakeholders, exploring ways to enhance patient care, community coordination and other health care issues.

  • The Social Worker completes patient record documentation on all cases according to established policies.


  • The Social Worker participates in interdisciplinary collaboration relevant to program development, grant writing, research initiatives, and teaching.

  • The Social Worker attends and participates in departmental, hospital, professional and relevant community meetings .and liaisons with community health and social service agencies. The Social Worker may participate in community coalitions and advisory boards.

  • The Social Worker participates in activities of the Social Work Division including but not limited to mandated training, health updates, time reporting and professional development.


  • The Social Worker provides training to dental residents and other EIOH staff regarding issues that impact care including social determinants of health, child abuse/neglect, and domestic violence . .


  • The Social Worker may collaborate with faculty, residents and administration on research projects related to the social determinants of health.

Professional Accountability, Supervision and Evaluation

Clinical and Administrative Accountability

The EIOH Social Worker is accountable administratively and clinically to the Social Work Associate Director for provision of social work services, and adherence to policies, and quality assurance practices of SMH Social Work Division. The EIOH social worker is administratively accountable to both the SMH Social Work Division and the Eastman Institute for Oral Health (via the Director of EIOH) and will provide social work services in conformance with policies, procedures and practice standards of EIOH.


The professional social work and competency assessment is conducted annually by the Social Work Associate Director, and includes feedback from designated EIOH personnel. Annual salary increase and promotional recommendations are subject to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

Position QualificationsThe Social Worker must hold a Master of Social Work degree from an accredited institution. Licensure as a social worker (LMSW), as defined by the State Board for Social Work, State Education Department Division of Professional Licensing Services is required within 18 months of employment. Relevant experience is preferred.
