GENERAL DESCRIPTION :

The gender wellness social worker provides psychosocial assessment and psychiatric diagnosis and treatment for transgender and gender diverse adult patients 18 years and older in the Adult Ambulatory Clinic. The gender wellness social worker functions as primary therapist and is responsible for the overall coordination and implementation of the treatment plan for patients assigned to their caseload. The gender wellness social worker participates on an interdisciplinary treatment team and develops liaisons with health and social agencies in the community, as well as with other health professionals within the hospital to support evaluation and treatment of the patient. The gender wellness social worker also provides treatment, and crisis intervention as appropriate. Gender wellness social workers stays informed about World Professional Association of Transgender Health Standards of Care and will work directly with the Clinical Director of Transgender Patient Experience.

SPECIFIC RESPONSIBILITIES :

Administrative (7%)

  • Participates in the development of a supervisory contract and in annual performance evaluation.

  • Attends Social Work Division and Program Staff Meetings.

  • Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice and patient care.

  • Participates in the routine committee process of the Social Work Division and complies with Social Work Division and Departmental quality assurance and staff development activities.

  • Participates in activities of the Social Work Division including, but not limited to timely completion of time reporting and other Division forms; Hospital and Division mandatory inservice training programs and UHS reviews.

  • Subscribes to the Departmental of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competency events and training appropriate to job duties.

  • Attends meetings linked to the developing Gender Health and Wellness interdisciplinary teams.

  • Participates in the developing Project ECHO (Extension for Community Healthcare Outcomes) to improve access to affirming care in the Greater Rochester Area.

  • Develops and disseminates community education

    Clinical (90%)

  • Performs psychosocial assessment and diagnostic formulations for patients and, when indicated, their families.

  • Provides social work treatment to patients and families within the Gender Wellness Program within Ambulatory Services

  • Ensures the development and implementation of comprehensive and individualized treatment plans for patients

  • Provides screening, evaluation and group treatment.

  • Functions collaboratively and consultatively as member of interdisciplinary team.

  • Coordinates with other resources in the community and within SMH to ensure continuity of care.

  • Documents treatment in patient's chart in compliance with Division, Department and Hospital policy.

  • Provides back up gender wellness social work services as requested and able to other unit/programs during the absence of certain co-workers.

    Teaching and/or Research (3%)

  • The gender wellness social worker may provide supervision and training to gender wellness social work students and colleagues as determined by the social work supervisor in consultation with the Clinical Manager for Social Work, the Director of Ambulatory Services, and the Clinical Director of Transgender Patient Experience.

  • The gender wellness social worker may participate in teaching concerned with issues of patient and family assessment, and treatment interventions as they relate to the psychosocial aspects of patient care within the developing Gender Wellness Program within Adult Ambulatory.

  • The gender wellness social worker may participate in interdisciplinary and/or social work research endeavors as determined by the social work supervisor in consultation with the Clinical Manager for Social Work, the Director of Ambulatory Services, and the Clinical Director of Transgender Patient Experience.

    PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION :

    Supervision/Consultation

    The gender wellness social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).

    Accountability

    The gender wellness social worker is accountable administratively and clinically to their social work supervisor (and through their Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description. The gender wellness social worker is accountable to the Director of Ambulatory Services and to the Clinical Director of Transgender Patient Experience for providing professional social work services in accordance with the policies and standards established by the Service and the Department of Psychiatry.

    Evaluation

    Social work evaluations are conducted annually by the gender wellness social worker's supervisor, with commentary from the Clinical Director of the Service and the Clinical Director of Transgender Patient Experience. Salary and promotional recommendations are made by the gender wellness social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to the guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

    POSITION QUALIFICATIONS :

    The gender wellness social worker that occupies this position must have a MSW from an accredited social work graduate program. They must be a licensed Social Worker, have at least 5 years of experience, and expertise in providing affirming care to transgender and gender diverse individuals.

    POSITION DESCRIPTION MAINTENANCE :

    This position description is reviewed annually by the gender wellness social worker, their supervisor, and the Clinical Director of Transgender Patient Experience. Position review provides opportunity for comment from the Director of the Service, and for updating, as needed. Similarly, if a new gender wellness social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of the Service and the Clinical Director of Transgender Patient Experience. Any position description modifications are authorized by the Social Work Division Clinical Manager for Psychiatry.
