The senior social worker provides support to the Program Director for the Adult Partial Hospital Programs and serves as a member of SW Executive Committee. In this role, the senior social worker is responsible for the clinical and administrative oversight of social workers assigned to the Adult Partial Hospital Programs, and may have supervisory oversight of staff members from other disciplines.
The senior social worker may also functions clinically as an Intake Coordinator/Therapist providing direct services to patients ages 18 and older within the Adult Partial Hospital Program (PHP) when clinical coverage is needed. The senior social worker’s clinical duties include psychosocial assessment, psychiatric evaluation, family assessment, crisis intervention, group treatment, treatment planning, and case coordination. All of these activities occur in collaboration with the patient’s attending physician and other members of the Interdisciplinary Treatment Team
Is a member of the interdisciplinary leadership team, which develops and implements policies, procedures, and programs for the Adult Partial Hospital Program.
Participates in the development and/or execution of quality improvement programs on the unit.
Supports Program Director in ensuring clinicians meet access standards, treatment planning and documentation standards and internal and external communication standards.
Ensures the provision of positive clinical outcomes, consumer and family satisfaction and seamless transition of patients to the next level of care as indicated.
Ensures supervision and professional development of social work and other clinical staff, in collaboration with Program Director
Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to patient care, discharge planning, and professional practice.
Participates in the recruitment of social work staff for the APHP.
Participates in the routine committee process of the Social Work Division and complies with Social Work Division and Department quality improvement and staff development activities.
Participates in activities of the Social Work Division including time reporting and other Hospital or Division mandatory in-service training programs and UHS review.
Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
Participates in available cultural competency events and training appropriate to job duties.
Functions as a clinician to program patients as needed. (Primary therapist, intake therapist, and/or group therapist)
Provides a comprehensive assessment of each patient as scheduled.
In the context of the Interdisciplinary Treatment Team, and in concert with the attending physician, develops a comprehensive treatment plan, and coordinates the delivery of services to the patient within the program.
Engages the patient’s family or other supports in the assessment and treatment process, and where appropriate provides education, support, and/or treatment to the patient’s family/support system.
Provide clinical supervision and oversight to clinical staff on individual therapy, group therapy, intake evaluation, intake coordination, and discharge planning.
Coordinates activities related to comprehensive discharge planning for patients on his/her caseload, and mobilizes family and community resources to support the patient’s successful functioning after discharge from the program.
Documents all patient care activities and interventions in the patient’s chart in compliance with Division, Department and Hospital policy.
Participates in administrative on-call activities of the Social Work Division, as appropriate.
Provides back-up clinical services as needed during the absence of program staff.
The senior social worker provides education to students of various disciplines shadowing at PHP.
The senior social worker may function as a fieldwork instructor for graduate and undergraduate social work students.
The senior social worker may participate in the development and/or implementation of social work or interdisciplinary research endeavors, as determined by the social work supervisor in consultation with the Social Work Manager in Psychiatry.
PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION:
The senior social worker’s professional performance is supervised by a Social
Work Division supervisor who is licensed by the State of New York (LCSW).
The senior social worker is accountable administratively and clinically to the Social Work Division Clinical Manager for Psychiatry for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The senior social worker’s job performance must conform to the expectation set forth in the position description.
The senior social worker is accountable to the Program Director for PHP and Adult Services Division Chief for operating the PHP in accordance with the standards established by the Department of Psychiatry, the Social Work Division, and Strong Memorial Hospital.
Social work evaluations are conducted annually by the senior social worker’s supervisor, with commentary from the Adult Services Division Chief. Salary and promotional recommendations are made by the social worker’s supervisor in consultation with appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester .
The senior social worker who occupies this position must have a MSW from an accredited social work graduate program and have a New York State license (LCSW).
POSITION DESCRIPTION MAINTENANCE:
This position description is reviewed annually by the senior social worker and his/her supervisor. Position review provides opportunity for comment from the Adult Services Division Chief, and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Adult Services Division Chief.
