    The social worker assigned to the Strong Ties Assertive Community Treatment (ACT) Team functions as an employment / vocational specialist to individuals ages 18 and older. The social worker is an integral member of the ACT Team, providing comprehensive mental health and dual diagnosis services for individuals who have serious and persistent mental illness and have not connected with traditional mental health outpatient clinics. The ACT Employment / Vocational Specialist works with clients to develop client-driven goals and creative ways to help clients meet these goals. Services are provided in settings where clients are most comfortable, mostly in clients’ own home. The vocational specialist receives training and support from other specialists on the ACT team in areas of substance abuse and rehabilitation, family therapy, mental health assessment and general health issues. He/she also provides cross training to other team members that helps ensure they are aware of and focused on helping client reach employment, vocational, and rehabilitation goals. As a team member the social worker is expected to function in a trans-disciplinary role.


Administrative (5%)

  • Attends Social Work Division, Social Work in Psychiatry and Program staff meetings as appropriate.

  • Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice and patient care.

  • Participates in routine committee process of the Social Work Division, and facilitates Division and Hospital quality assurance and staff development activities.

  • Participates in activities of the Social Work Division including, but not limited to time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.

  • Subscribes to the Departmental of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competency events and training appropriate to job duties.

Clinical (95%)

  • Provides cognitive behavioral therapy for psychosis including education about stress-vulnerability and symptom management. Develops interventions based on the OMH Illness Management and Recovery (IMR) Toolkit, the OMH Wellness Self-Management curriculum and the OMH IDDT approach.

  • Assists clients in maintaining themselves in the community, including finding and furnishing apartments, setting up financial budgets, applying for entitlements, and learning skills of daily living.

  • Transports clients as needed to appointments and assists clients in moving or shopping.

  • Participates in a client on-call phone rotation with other ACT team members once every five weeks.

  • Works with clients individually and in group format to help initiate and maintain employment /vocational rehabilitation.

  • Provides feedback to the treatment team regarding patient progress in recovery and facilitates and supervises the placement of patient in other community treatment programs as necessary.

  • Engages clients in both individual and group psychoeducational to minimize the incidence of relapse using recovery-based principles, collaboration and shared decision making.

  • Prepares a written assessment of client skills, deficits and growth potential

  • Serves as liaison between ACT program and other community agencies that provide employment/vocational training and related services for clients who choose to avail themselves of additional services/opportunities.

  • Administer breathalyzer tests and obtain toxicology samples per client’s AOT court order. Provide weekly reports to AOT for those on your caseload who have an AOT court order.

  • Documents treatment in patient’s chart in compliance with Division, Department, and Hospital Policy.

  • May provide backup social work services as requested to other units/programs during the absence of certain co-workers.


  • Based on experience and as determined by the social work supervisor, the social worker may supervise social work graduate students. Upon invitation from the Program Director and in consultation with the social work supervisor, he/she may offer learning experiences for psychiatric residence, medical students, and family medicine residence. Supervision relates to psychosocial assessment and intervention with patients and their families.


  • Based on experience/interest, may participate in interdisciplinary research as determined by the Social Work Supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.


Supervision/ Consultation

The social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).


  • The social worker is accountable clinically and administratively and to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The senior social worker's job performance must conform to the expectations set forth in the position description.

  • The social worker is accountable to the ACT Team Leader for providing professional social work services in accordance with the policies and standards established by the Service and the Department of Psychiatry.


Social work evaluations are conducted annually by the social worker's supervisor with commentary from the ACT Team Leader and/or designee. Annual salary and promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and professional promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.


The social worker who occupies this position must have a MSW from an accredited social work graduate program and a New York State social work license. This position also requires experience in at least two years’ experience working with the SPMI population or an equivalent combination of education and experience. Must possess a valid New York State Driver's license and a satisfactory driving record.


This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the ACT Team Leader, and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the ACT Team Leader. Any position modification is authorized by the Social Work Division Clinical Manager for Psychiatry.
