Responsibilities

GENERAL DESCRIPTION:

The social work assistant functions as a professional member of the Department of Psychiatry’s Rochester Community Mobile Crisis Team (MCT) serving patients in crisis in a variety of settings across Monroe County. Provides expertise with regard to community resources, supports and agencies, and assures continuity of care via direct and indirect means of communication with persons involved in the care of the patient.The social work assistant participates in the planning and delivery of appropriate service linkages, and serves as liaison and advocate for patients and their families with identified service providers. The social work assistant meets with patients and families to assist with the collection of psychosocial data and collaborates in the evaluation and referral planning processes. The social work assistant participates on the interdisciplinary treatment team for the patients on his/her caseload. The social work assistant, in collaboration with licensed clinical staff, participates in crisis intervention, and assessment and management of patient safety. The social work assistant serves as a consultant to members of the interdisciplinary team regarding child abuse, domestic violence, and entitlement programs. The social work assistant may also provide psycho-educational services to the patient/family. The social work assistant develops and maintains relationships with community agencies and providers who refer to and receive patients from the MCT. The social work assistant may provide oversight, supervision, and consultation for BSW interns, with supervisor approval.

Responsibilities:

Administrative and Supervisory:

  • Ensures that Hospital, Department, and Social Work Division administrative and clinical policies and procedures are implemented with regard to patient care, discharge planning and professional responsibility.

  • Participates in activities of the Social Work Division including, but not limited to, timely completion of time reporting and other Hospital or Division forms, Hospital and Division mandatory inservice training programs and UHS reviews.

  • Provides assistance and consultation to the staff of MCT in regards to referral planning and critical resources.

  • Attends Social Work Division and Program Staff meetings.

  • Participates in the routine committee process of the Social Work Division and complies with Social Work Division and Departmental quality assurance and staff development activities.

  • Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competency events and training appropriate to job duties.

Clinical:

  • Assists in the performance of psychosocial evaluation of patients and their families.

  • Functions as a member of the interdisciplinary treatment team for each patient assigned to his/her caseload. Consults and collaborates with team members and patient/family on the development of an appropriate discharge/follow up plan.

  • Assists the Crisis Therapists and providers with agency contact both to collect data and to assess agency involvement.

  • Coordinates the referral process to appropriate community resources to ensure that the discharge plan is implemented in the patient’s discharge from the service.

  • Provides social work follow-up to patients, families and agencies as needed after discharge to ensure continuity of care and integrity of the discharge plan.

  • May facilitate psycho-education with patients and families of the MCT.

  • Assists and supports patients and their families in their efforts to interact with the complex social service agencies involved in their care.

  • Assists patient and families in interfacing with the hospital system and the finance office.

  • Documents social work interventions in the patient’s chart in compliance with Division, Department and Hospital policy.

  • Provides additional Social Work services as requested by supervisor.

PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION:

Supervision/Consultation:

  • The social work assistant’s professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).

    Accountability:

  • The social work assistant is accountable administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to social work policies, procedures, and standards of professional conduct. The social work assistant’s job performance must conform to the expectation set forth in the position description.

    • The social work assistant is accountable to the MCT Medical Director and MCT Director for providing professional social work services in accordance with the policies and standards established by the Department of Psychiatry and the Hospital.

Evaluation:

  • Social work evaluations are conducted annually by the social work assistant’s supervisor. Annual salary and promotional recommendations are made by the social worker assistant’s supervisor in consultation with appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

POSITION QUALIFICATIONS:

The social work assistant who occupies this position must have a Bachelor of Social Work degree from an accredited social work undergraduate program.

POSITION DESCRIPTION MAINTENANCE:

This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Director of Mobile Crisis Team, and for updating, as needed. Similarly, if a new social work assistant is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with Medical and Clinical Directors of the MCT. Any position description modification is authorized by the Social Work Division Clinical Manager for Psychiatry.
