Description of Patient Population:

GENERAL DESCRIPTION:

The social work assistant is responsible for the implementation of phone triage and psychosocial assistance for patients aged 18 yrs. and older and for ensuring continuity of care upon discharge for the Adult Ambulatory Services. He/she participates in the planning and delivery of appropriate service linkages, and serves as liaison and advocate for patients and their families with identified service providers. He/she is a member of the interdisciplinary treatment team for the patients.

Administrative (5%)

  • Ensures that Hospital, Department, and Social Work Division administrative and clinical policies and procedures are implemented with regard to patient care, discharge planning and professional responsibility.

  • Participates in activities of the Social Work Division including completion of time reporting and other Hospital or Division forms, Hospital and Division mandatory inservice training programs and UHS reviews.

  • Attends Social Work Division and Program Staff meetings as appropriate.

  • Participates in the routine committee process of the Social Work Division and complies with Social Work Division and Departmental quality assurance and staff development activities.

  • Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  • Participates in available cultural competence events and training appropriate to job duties.

Clinical (95%)

  • Conducts telephone screening on new referrals to ensure appropriateness for clinic services.

  • Subsequent to phone screening, assigns patient to most appropriate intake slot. May administer screening tools and conduct record review.

  • May assists the social worker (MSW) with agency contacts both to collect data and to assess agency involvement.

  • May provide case management services to active clinic patients.

  • Carries COPS/Priority pager as necessary.

  • Monitors requests for patient records.

  • Documents social work interventions in the patient’s chart in compliance with Division, Department and Hospital policy.

  • Provides back up Social Work services as requested in the absence of co-workers

Teaching (TBD)

  • The social work assistant may function as a field work instructor for undergraduate social work students. He/she may provide education to medical students regarding the role of social work and resources available in the community.

Research (TBD)

The social work assistant may participate in social work and/or interdisciplinary research endeavors, as determined by the Social Work Supervisor in consultation with Clinical Manager in Psychiatry.

PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION:

Supervision/Consultation

The social work assistant’s professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LMSW).

Accountability

  • The social work assistant is accountable administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The social work assistant’s job performance must conform to the expectation set forth in the position description.

  • The social work assistant is accountable to the Director of Adult Ambulatory Services for providing professional social work services in accordance with the policies and standards established by the Department of Psychiatry and the Hospital.

Evaluation

Social work evaluations are conducted annually by the social worker assistant’s supervisor, with commentary from the Director of Ambulatory Services. Annual salary and promotional recommendations are made by the social worker assistant’s supervisor in consultation with appropriate Social Work Division personnel. Salary increases and promotional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

POSITION QUALIFICATIONS:

The social work assistant who occupies this position must have a BSW from an accredited social work undergraduate program.

POSITION DESCRIPTION MAINTENANCE:

This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Director of Adult Ambulatory Services, and for updating, as needed. Similarly, if a new social work assistant is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Director of Acute Services. Any position description modification is authorized by the Social Work Division Clinical Manager for Psychiatry.
