Senior Social Worker

Responsibilities

General Purpose:

Provides professional social work services to patients and their families; collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients; participates in formulating procedures and policies of the Division of Social Work; and participates in educational and research programs, including the orientation and training of new staff and students.

Responsibilities:

Evaluates the psycho-social status of patients and family to determine specific needs for service. Provides brief or intensive assistance through counseling and direct services to the patient and patient's family. Discusses plans for treatment and disposition with patients; helps patients and family to cope with home and community problems which hinder health care; evaluates and makes recommendations for home care, financial assistance, use of clinics, and rehabilitation services. Makes home visits when necessary. Refers patient and family to community resources to maintain continuity of service.

The senior social worker is responsible for the professional supervision of the social work staff in the assigned clinical areas. The senior social worker carries out administrative, teaching and consultation responsibilities associated with the Social Work and PM&R programs. The senior social worker is responsible for the development and coordination of social work services in collaboration with the Clinical Manager and administrative staff in the clinical areas assigned.

Participates in recruitment and training of social workers in assigned clinical area

Ensures implementation of the Social Work Division annual performance evaluation, including competency assessments for social work staff assigned to the clinical area.

Establishes and maintains cooperative working relationships with community agencies which may be helpful to patients, such as BIANYS, TBI Waiver Program, Bridges for

Brain Injury, CDPAS, etc.

Provides social work consultation to other members of the department of PM&R. Attends rounds and staff meetings to coordinate the efforts of the hospital and clinic staffs for the patient's welfare. Helps develop a rehabilitation plan for individuals through the sharing of knowledge of the patients and mobilization of available resources.

Attends and participates in departmental, hospital, professional and special meetings to keep abreast of social work activities and to share new developments in the Strong Memorial Hospital Social Work program. Meets with members of community agencies to share information, procedures and techniques. Maintains a thorough knowledge of social work practices and the policies of community agencies through workshops, symposia and social work literature.

Makes recommendations on procedures and policies of the Division of Social Work. Completes documentation on all cases according to established policies. Conducts correspondence with other institutions and community agencies.

Qualifications:

Minimum Education

• Master’s degree from an accredited school of social work

Experience

• 2 years professional social work practice beyond the Master's degree or equivalent combination of education and experience.

• Brain Injury and Spinal Cord Injury patient population experience

Knowledge, Skills & Abilities

• Effective organization and communication skills.

• Ability to coordinate and manage a team.

Certification

• Licensure as a Master Social Worker (LMSW) as defined by the State Board for Social Work, State Education Department, Division of Professional Licensing Services within 18 months of employment

Preferred:

• 8+ years of relevant experience

• Experience educating others
