Rehabilitation Therapist

GENERAL PURPOSE:

The Rehabilitation Therapist (RT) position is primarily responsible for providing Targeted Case Management and Psychiatric Rehabilitation Services across all Strong Recovery clinics. This hybrid model of care is intended to support the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. The CCHBC RT will provide case management services to support patients around goals aimed at providing access to services that encourage individuals to resolve problems that interfere with their attainment or maintenance of independence, self-sufficiency, or maintaining themselves in the community. Additionally, the RT performs psychosocial rehabilitation services designed to assist the individual to overcome mental health and substance use barriers that may have interfered with the person’s ability to function independently and perform normative adult roles in the community. The intent of this role is to assist the individual to restore the individual’s functional level to the fullest possible. The primary focus of these services with the individuals includes assistance with vocational and educational goals.

Under general direction and with considerable independence, performs several care management and psychiatric rehabilitation services consistent with the CCBHC and also the Department of Psychiatry Policy. Establishes and maintains cooperative working relationships with community providers and resources to obtain needed services and support for enrolled patients. Works as part of an interdisciplinary team to support the care of patients enrolled in a designated CCBHC outpatient clinic.

SPECIFIC RESPONSIBILITIES :

  • Conducts assessment of identified patients for psychiatric rehabilitation services.

  • Provide targeted case management services

  • Serves as a liaison for the treatment team to support activities to engage patients in care, connection to care management, vocation and education resources and ensures basic necessities are met. Utilizes community and family resources to create sustainable support systems for patients. Reviews and discusses plan with patient and the treatment team, focusing on linking individuals to clinical and social services within the system and community providers.

  • Designs appropriate treatment plan goals according to patients’ psychiatric rehabilitation needs.

  • Conducts evidence-informed treatment with patients

  • Assesses successful completion of goals and plans termination of psychiatric rehab services accordingly.

  • Available to triage case management needs for CCBHC program recipients on an urgent basis, including walk-in’s.

  • Personally committed to understanding and meeting the unique needs of external and internal customers.

  • Contributes to the vision and mission of the CCBHC.

  • Maintains a thorough knowledge of Department of Psychiatry, CCBHC related policies and procedures.

  • Coordinates with support staff regarding patient scheduling, fees, and insurance issues.

  • Promote hope and recovery by using strengths-based, culturally appropriate, and person-centered practices.

  • Collaborates with outpatient team members to provide coordinated care

  • Available to assist clinicians as needed, to support case management needs.

  • Establishes communication using established rules of confidentiality and laws concerning rights of patients, and uses correspondence with family, other health-related agencies, employers, criminal justice programs, and social service status.

  • Maintains active case records including written treatment plans, problem-oriented progress notes, periodic updates of treatment plans, and appropriate documentation of incidents, discharge summaries, interagency.

  • Completes and maintains professional documentation.

  • Responsible for documentation of services provided in the patient’s electronic health record.

  • Maintains professional standards by reading, attending seminars, workshops and conferences.

  • Attends weekly clinical/training/supervision meetings.

  • Participates in Departmental and programmatic diversity presentations and meets annual training expectations on diversity

Other duties as assigned, including but not limited to:

  • Milieu management in clinic area

  • Provides milieu management in Reception areas, supporting patients to engage with mental health care, connecting patients to Health Home Care Management Services and other support services.

  • Toxicology screen monitoring.

  • Assess patients for physical, behavioral signs of drug/alcohol use and perform

  • BAC tests as needed.

  • Assist staff in assessing patients’ behaviors, and develop a plan of action to help maintain the safety of other patients and staff in clinic.

REQUIREMENTS:

  • Bachelor’s degree with major course work in a related health/human service/occupational field required.

  • Master’s degree in an addiction/mental health field are preferred.

  • 2-3 years of relevant experience required.

  • Experience working with adults and/or children and adolescents regarding substance use/mental health disorder concerns, or an equivalent combination of education and experience preferred.

  • Experience/Education focused on assisting individuals with vocation/education goals

  • Must possess and maintain a valid New York State License, have a satisfactory driving record strongly preferred.
