The mental health coordinator is an integral member of the UR primary care site team. He/she provides screening, psychosocial assessment, psychiatric diagno'sis and brief, evidence based treatment for a primary care practice. In this role, the mental health coordinator also helps to identify high risk patients with behavioral health and medical comorbidities and contributes to a unified person-centered treatment plan with the goal of improving the patient's overall health and well-being, as well as facilitates access to specialty psychiatric and substance use care. The position will require formal and informal collaboration with primary care physicians, nurse practitioners and nursing care managers.
This position will be based at the Complex Care Center. The Center is an interdisciplinary primary care site targeting adults with pediatric onset conditions with considerable medical complexity.
The Center, located at 905 Culver Road, is a comprehensive resource for adults with childhood onset conditions including, but not limited to intellectual and developmental disabilities, cystic fibrosis, sickle cell disease, spina bifida, metabolic disorders, congenital heart disease, childhood cancers, myotonic dystrophy, and cerebral palsy. These patient populations have unique behavioral health challenges and needs including large caregiver circles, impact of medical illness on cognitive and emotional functioning, early engagement with the health care system, confronting mortality at a young age, among others. The rates of mood disorders and substance misuse are higher in th1s population than in the general medical primary care clinic sites.
Participates in the development of a supervisory contract and m annual performance evaluations.
Attends discipline-specific,Division,ProgramStaffandTeamMeetingsas indicated
Ensures that discipline, Division and Hospital policies and proceduresare implemented with regard to professional practice and patient care.
Participates in activities of the discipline including timely completion of time reporting and other Divisionforms; Hospital and Division mandatoryin-service training programs and chart reviews.
Subscribes to the UR Medicine goal of creating aculturallycompetent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
Provides child/adult protective consultation to primary care clinicians.
Participates in available cultural competency events and training appropriate to job duties.
Assessment and diagnostic interviewing
As a member of the primary care team works with PCPs to mutually enhance understanding of the patient/family, provides decision support fortreatment planningand assists in the creation, implementation and monitoring of biopsychosocial treatment plans.
Performs diagnostic and functional evaluations and makes recommendations to the primary care provider concerning the patient's treatment plan and goals.
Documents all assessment and treatment in patient's electronic medical record in compliance with Division and Hospital policy.
Evidence-based treatment and service delivery
Offers longitudinal interventions with patients including functional and strength based assessment and diagnosis; psychoeducation for patients and their support systems; medication adherence; motivational interviewing; and brief problem solving interventions aimed at promoting self-efficacy, enhanced decision making, and behavior change.
Initiates follow up to ascertain how patients are doing and to determine if changes in the treatment approach are indicated.
Supports efforts for 100% behavioral health screening for depression, anxiety and substance use.
Assists in tracking and monitoring, including tracking patient's adherence to treatment plans, reported outcomes tools, and status after referral to specialty mental health.
Participates as a member of the clinical primary care team and attends meetings.
Provides and participates in clinician/staff training and education in integrated behavioral health primary care model.
Provides back up mental health service coverage to other integrated therapists as needed.
Linkage with other programs
Facilities referrals to specialty behavioral health care; wellness programs; and community agencies.
Recommends and facilitates which patients should be referred to a specialty mental health facility.
- Provides Crisis Intervention during working hours.
The clinician that occupies this position has a master's degree in a mental health field from an accredited graduate program. They must be licensed by the State of New York (ie LMSW, UvfHC, LMFT) and competent in providing appropriate cognitive and behavioral interventions to individuals, families, and groups. Training and clinical experience in primary care/integrated behavioral health and collaboration preferred. Experience working with intellectual and developmental disabilities, substance use, and families is highly desirable.
