University of Rochester Kidney Transplant Coordinator - Nurse Practitioner/Physician Assistant in Rochester, New York
FT position available M-F, with some rotating call (call taken from home/setting up kidney transplant)
Exciting position in the solid organ transplant program. The focus of this position is on managing post-transplant kidney recipients and completing H&Ps; on pre-transplant kidney evaluations. Establishing good rapport with patients, their families, and referring physicians is key to this position. The ability to work in a fast paced environment while juggling multiple responsibilities is paramount. Strong clinical skills are required as the position has a subset of patients that are followed primarily by the NP/PA with attending coverage for both clinic and follow up. Working with pre-transplant RN coordinators to complete H&Ps; on kidney evaluations looking for kidney transplant is also included. Candidate must have excellent communication skills and be comfortable working in a dynamic multidisciplinary setting. Attendance to department and service weekly and monthly meetings is expected. Involvement in committees, developing and maintaining policy and procedures is encouraged. The candidate will serve as a resource to internal and external health care providers. The coordinator is responsible for adherence to hospital and APP policies, procedures and practice standards, education of patients and staff, assurance of service excellence, completion of state and hospital required reviews of practice and participation in performance and quality improvement and other activities as required.
Requires a Master's Degree in Nursing or related field and an active New York State license in good standing. Applicants will have 5 years experience as an RN with one to three years of leadership experience; or equivalent combination of education and experience. The Nurse Practitioner influences the quality and delivery of care in various roles including consultant, researcher, mentor, and practitioner. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
Completion of an AMA approved Physician Assistant program, licensure in good standing, and certified as a Registered Physician Assistant. Demonstrated clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
