Home Infusion Care Coordinator - Registered Nurse, Level II

Responsibilities

UR Home Infusion is seeking a team focused Registered Nurse to coordinate home infusion services. This is an exciting opportunity to collaborate nursing with the UR Home Infusion pharmacy team. The candidate must demonstrate the ability to function in a fast-paced environment, be autonomous in decision making, and have excellent assessment skills. As a small team, this candidate must have the ability to work collaboratively with all members of the healthcare team. They must be flexible and adapt readily to changing responsibilities.

Services include direct communication with hospital providers, staff nurses, coordination with patients and families, collaboration with referring community practices, education and onboarding new patients.

Qualifications

Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 6-12 months of RN experience. This position assumes the charge nurse role, actively participates in unit/service activities, supports co-workers, and assesses unit needs.

Pay Range

Pay Range: $ 33.00 - $ 42.90 Hourly

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
