Responsibilities

GENERAL PURPOSE:

Provides professional comprehensive care management services to patients of the Strong Behavioral Health, Care Management Program. Collaborates with health, behavioral health and social service providers and is responsible for the development and management of service plans for patients enrolled in a NYS-licensed Health Home who have need for the most complex, high utilizing and costly services and may be at high risk for under-utilizing services.

SPECIFIC RESPONSIBILITIES:

The Care Manager-Team Leader provides leadership and supervision to an assigned team of Care Managers: (100%)

Who together provide professional Comprehensive Care Management services to patients assigned to the Strong Behavioral Health Care Management Agency, by the Greater Rochester Health Home Network, Health Homes of Upstate New York and the Monroe County Office of Mental Health.

Provides ongoing clinical and performance supervision for Health Home Care Managers assigned to their team. Monitors team and individual productivity. Completes Annual Performance Evaluations.

Provides consult and direction to the team and individual team members to ensure efficient delivery of timely, appropriate and effective services to the clients on their caseload.

Oversees and completes incident reporting for themselves and care managers assigned to their teams as per hospital and NYS DOH Health Home policy.

The Health Home Care Management Team Leader is also responsible for auditing charts per the prescribed auditing policies and procedures and is responsible for making sure Health Home care management electronic documentation remains in compliance with NYS DOH Health Home policy standards. Team Leaders are responsible for conducting in-services in collaboration with the Director of Care Management on a variety of training topics including specific Health Home Care Management policies and procedures in addition to general care management practices, community resources, etc.

The Health Home Care Management Team Leader is responsible for Monitoring caseload sizes, providing accurate enrollment data to the intake coordinator, and coordinating/providing caseload coverage for members of their team when out of the office. The Health Home Care Management-Team Leader is also responsible for the approval, provision, and administration of WRAP funds to Health Home members assigned to care managers on their team. The Health Home Care Management Team Leader is also expected to participate in the ongoing recruitment of new Health Home Care managers.

Reports to the Strong Behavioral Health program Director of Health Home Care Management.

The Health Home Care Management team leader as part of their supervision responsibilities, will also provide Health Home Care Management services to Health Home members as part of coverage and support for their team when necessary. In addition to their supervisory responsibilities, under general direction and with considerable independence, performs complex care management services consistent with NYS Regulations and Policy for the provision of Health Home Services.

Establishes and maintains cooperative working relationships with community providers and resources to obtain needed services and support for enrolled Health Home members.

Tracks and monitors that the services connected to the client are being delivered as planned and are efficacious.

Utilizes community and family resources to create sustainable support systems for patients. Reviews and discusses plan with patient, focusing on linking individuals to clinical and social services with system and community providers.

Monitors utilization of services and encourages enrollees to follow treatment recommendations, ensures that care is assessable, attended and effective. Monitors the use of costly Emergency and Inpatient services, supports patient in keeping all aftercare appointments and addressing barriers as needed.

Completes documentation of services provided in eRecord, prescribed Care Management software, NYS OMH CAIRS software, and NYS HCS system, in compliance with hospital policy, NYS DOH Health Home regulation, and NYS OMH regulations.

Performs periodic review and update of Comprehensive Service Plans as prescribed by NYS DOH. Assists with peer record reviews and quality initiatives designed to improve quality of care and efficiency of service.

Each team leader is also assigned responsibility for program specific tasks in conjunction with the program Director to include managing crisis apartments, making sure program is in compliance with Assisted Outpatient Treatment and Health Home Plus state and local requirements, quality assurance reporting, training designations through Health Homes of Upstate New York as program champions, and overseeing the HARP and Home and Community Based Services program requirements mandated through the state Department of Health, Managed Care Organizations and Health Homes.

REQUIREMENTS:

Bachelor’s Degree required in an appropriate human services field required and 4 years direct experience working with individuals diagnosed with Serious Mental Illness (SMI); or an equivalent combination of education and experience. Master’s Degree Preferred. Must possess and maintain a valid New York State License, have a satisfactory driving record and have access to an automobile.
