GI Nurse Care Coordinator - Hepatology - Registered Nurse, Level III

Responsibilities

URMC GI Practice is in search of a driven, team-oriented nurse to join our multidisciplinary team providing focused, sub-specialty care to our hepatology patient population. This role provides nurse case management for patients with complex medical needs in management of cirrhosis, Autoimmune hepatitis and other complex liver disease. Nurse Care Coordinator will develop and implement a care management plan based on patient/provider developed goals and objectives to promote improved health care outcomes and quality of life. This role involves managing a list of high-risk hepatology patients, assisting with coordination of appointments with various departments such as Transplant, IR, Oncology, Nutrition, infusion, etc. The Care Coordinator will assist in clinic providing education and support, and telephone triage/ in-basket management. The ideal candidate will have a Bachelor’s degree and 2-3 years GI nursing experience with strong organizational and communication skills, as well as experience working within the electronic medical record.

Qualifications

Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 3 years of RN experience. This position utilizes expert skill and knowledge to deliver nursing care, directs others, and assumes the role as a care coordinator or preceptor.

Pay Range

Pay Range: $ 36.37 - $ 47.28 Hourly

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
