Full Time 40 hours Grade 007 Urology M&D;
8 AM-4:30 PM; OCC WKENDS
With demonstrated ability to work with limited supervision, with flexibility to vary specific tasks as needed and coordinate with other data management staff to accommodate project deadlines, perform data abstraction and data entry from medical record documentation; maintain strict adherence to federal HIPAA policies and departmental data quality assurance and quality control standards; provide electronic data summaries as requested; assist in preparation of periodic reports as needed; assist in training of students or other staff as needed.
Perform abstraction of data from source records in patience records (electronic, paper source documents), data collection forms as directed. Involves gaining familiarity with and reviewing documentation in context of primary treatment, regularly associated medical procedures and typical outpatient follow up visits related to medical condition.
Enter data into the relevant data collection format or program (e.g. web application, Microsoft Excel, Microsoft Access) efficiently to complete assigned tasks.
Identify and follow up on data requiring clinician review/interpretation. Document resolution to questions and relevant details. Note required data elements not found during record review. Follow up as directed to determine if additional source documentation available and to ensure issue addressed.
Perform self-audit at all phases of the data collection, data entry and data reporting to ensure that data collected is complete and accurate (e.g. no typographical errors, consistent with source document). Report deviations of data entry/collection as appropriate, to supervisor.
Compile data, review computerized data queries to identify missing data fields and assist in consolidating/analyzing data as directed. Ensure data compiled is clear, any requested reports are accurate and meet all necessary report requirements.
Advise and assist with design of tools (e.g. forms, schedules, tracking logs) to ensure data collection is performed within established timeframes, or as directed by supervisor.
Retrieval/return of patient records used for data extraction; maintenance of tracking log of records loaned (from the department files or HIMS) for review.
Requires 1 year of relevant post High School education or equivalent and 1-2 years of related experience.
Bachelor’s degree preferred with coursework in biological or biomedical sciences or an equivalent combination of education and experience.
Requires strong computer skills (MS Access, MS Excel and MS Word, or similar applications for database management, spreadsheet and word processing). Accuracy and close attention to detail and excellent organizational skills are required.
Knowledge of medical terminology and interest in medical science strongly desired.
