Advanced Heart Failure/ Post Transplant - Transplant Nurse Coordinator

Responsibilities

Under the general guidance of the practice physicians and advanced practice providers, the transplant coordinator is primarily responsible for maintaining standards of care, patient satisfaction and coordination of both listed and post-heart transplant care. Responsibilities include direct patient care, patient education, managing waitlist with the APPs in clinic, discharge planning, and navigating safe transitions of care. Serves as important liaison for the interdisciplinary team and community agencies. Participates in quality improvement activities and ensures adherence with transplant regulatory requirements. Maintains current knowledge of medical diagnoses and therapeutics in the subspecialties of cardiology and heart transplant. Current Registered Nurse licensure, ACLS and CPR Certifications required. Requires excellent clinical, communication and organizational skills. Requires multitasking and critical analysis. Must be able to function independently within the limits of the position. Demonstrated ability to work as a member of a team.

Pay Range

Pay Range: $38.26 - $49.74

The referenced pay range represents the minimum and maximum annual salary for this faculty position. Individual salaries will be determined within the job’s salary range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Incentive-based compensation may also be applicable and will be discussed during the hiring process.
