FT (40 hours) position available: Monday – Friday days with some late shifts and occasional Saturdays.

Responsibilities

Golisano Childrens Hospital’s Pediatric Practice is in search of a team oriented nurse with pediatric experience to join our multidisciplinary team providing family centered primary care to a diverse population of patients and families. Seeking a strong candidate with a career focused on promoting health equity and inclusion. This role provides nurse case management for patients with complex medical needs by developing and implementing a care management plan based on patient/caregiver/provider developed goals and preferences, and disease states to promote improved health care outcomes and quality of life. The ideal candidate will have a Bachelor’s degree and 2-3 years pediatric nursing experience with strong organizational and communication skills as well as experience working within the electronic medical record. The nurse case manager will work effectively as a member of the health care team by collaborating on population health management, complex care and transitional care management. Sign Language or Spanish fluency preferred but not required.

Qualifications

Requires a Diploma, Associate or Bachelor's degree from an accredited Nursing Program and an active New York State RN license in good standing. Applicants will have a minimum of 6-12 months of RN experience. This position assumes the charge nurse role, actively participates in unit/service activities, supports co-workers, and assesses unit needs.
