Part Time 20 hours Grade 053 Psychiatry SMH Crisis
The social worker assigned dot the Comprehensive Psychiatric Emergency Program (CPEP) functions as a primary evaluator and/or complex discharge planner as dictated by the needs of patients present in CPEP. Primary Evaluator functions include completion of the social work psychosocial risk evaluation and psychiatric evaluation. The social worker also provides crisis intervention and ensures continuity of care between CPEP and inpatient or outpatient providers, resource consultation, and routine discharge planning for patients they evaluate. The function of Complex Disposition Planner is assigned when nonsocial work evaluators identify complex discharge needs for a patient they are evaluating. Functions include: identification and intervention around individual and systemic barriers to discharge; collaboration with other agencies, services, and community providers as needed towards achieving discharge; notification and involvement of administrative personnel as needed to achieve a safe discharge.
The social worker who occupies this position must have an MSW from an accredited social work graduate program and must obtain New York State licensure with eighteen months of employment.
