Job Summary

Under guidance from the director and with latitude for some judgment, the Patient Navigator will be responsible for implementing the National Accreditation Program for Breast Cancer (NAPBC) Patient Navigation Project. This project will help underserved women, regardless of their insurance status, navigate the healthcare system to complete breast cancer screening according to clinical guidelines. The Patient Navigator will engage at least one (1) community organization and at least one (1) affiliated health system in each of 6 counties (Monroe, Livingston, Wyoming, Alleghany, Steuben and Chemung) to refer eligible women in need of breast cancer screening for navigation support to and through breast cancer screening. The Patient Navigator is responsible for contacting at least 800 women annually to offer patient navigation services.

Specific Responsibilities:

Health Systems. Work with at least one affiliated health system (insurance carrier, hospital, FQHC, and/or clinicians), either in or across counties, to identify patients in need of routine comprehensive breast screenings. Receive and assess data feeds to determine patients’ need for navigation. Contact referrals to offer patient navigation.

Community Partner. Establish referral agreements with at least one community partner (may include local peer education program, health center and/or health plan) in each of 6 counties who will provide referrals to women from the identified priority population(s). Track number of patients referred from partner organizations. Contact referrals to offer patient navigation.

Patient Navigation. Contact at least 800 women annually and assess screening status. Offer screening and navigation/supportive services to patients in need. Track screening test completion. Monitor activities related to positive findings, diagnostic and/or treatment services.

Project Monitoring. Create process to track and monitor number of patients referred, contacted, navigated, screened and any activities related to positive findings. Prepare analytical reports of project progress for interim, quarterly and annual reporting.


Bachelor’s degree with major course work, in an appropriate health, social or technical field, and 1 year of experience in human subject research coordination (prefer 3-5 years of related experience); or an equivalent combination of education and experience. Background in social work and/or social services preferred. Excellent oral and written communication skills; the ability to interact effectively with a wide range of individuals, including patients, peers, clinicians/researchers at all levels and staff in a variety of office settings; expertise with social science research methods, advanced knowledge of Microsoft Office Suite, Internet search techniques, and basic knowledge of medical terminology and ability to learn and master new software quickly.
