Overview:

The DSRIP Project Manager position will represent the DSRIP Program across the Primary Care Network and provide overall leadership and project administration to internal DSRIP Primary Care identified locations and teams. The project manager will collaborate with the DSRIP Program Office located within the University of Rochester Strong Memorial Hospital and affiliates for the Regional DSRIP initiative. The goal of the DSRIP Project Manager is to support the implementation and reporting of clinical and non-clinical programs as a part of the UR Medicine DSRIP initiative and other activities related to system reform to meet value-based contracting opportunities.

This role requires a developed understanding of clinical and administrative processes, an ability to manage and coordinate multi-faceted projects across DSRIP projects in the Primary Care Network, and the ability to develop consensus across multiple stakeholders in an academic medical center setting. The Primary Care DSRIP Project Manager will work closely with the PCN DSRIP Population Health Director and Primary Care Leadership Team to oversee the development and execution of program initiatives based upon identified and emerging needs. The Project Manager is also responsible for producing reports and presentations for the Primary Care Network and the DSRIP Program Office. This role will build and maintain interdepartmental relationships and work closely with the DSRIP Program and project-level teams to plan and executive activities related to Medicaid system delivery reform.

Responsibilities:

Under general direction and with considerable latitude for the exercise of independent judgment and initiative:

DSRIP Project Administration and Coordination– In collaboration with the PCN Leadership Team, will manage and coordinate DSRIP projects across a multidisciplinary team comprised of clinicians, administrators and operational support staff working in Primary Care identified DSRIP practices. Leads and advises the development, implementation, and evaluation of designated projects related to the DSRIP Program within the Primary Care Network. Works to plan and executive activities related to reform of Medicaid delivery systems. Collaborates across DSRIP project teams and with other organizational work groups, committees and councils.

DSRIP Project Evaluation – Monitors progress of DSRIP project activities, ensures all critical implementation criteria are met and provides status reports to DSRIP Program leadership, PCN leadership, and PCN DSRIP sites. On a regular basis collects and analyzes information related to DSRIP projects in primary care. On a regular and on-going basis, analyzes and interprets DSRIP program information, performs appropriate drill down queries, summarizes and presents findings and opportunities.

Population Health Initiatives– Develops and oversees initiatives to improve DSRIP and Medicaid population heath quality metrics at DSRIP practices and other sites in the primary care network with significant Medicaid populations. Monitors population health quality metrics and routinely manages quality improvement interventions to improve quality programs.

Communication/Liaison- Serves as liaison between DSRIP program within Primary Care and DSRIP Program Office. Develops and executes a project communication plan ensuring clear and effective communication to project team and across the Primary Care project teams. Maintains continuous feedback loop between projects and DSRIP Program Office. Troubleshoots breakdowns in communication in a timely manner as issues arise.

*Requirements: *

Bachelor's degree in Healthcare Administration, or a related field with; 5 years of prior health project coordinator or healthcare management level experience or an equivalent combination of education and experience; knowledge of Project Management concepts and principles preferred.

Excellent verbal and written communication and interpersonal skills, proficiency and ability to learn new software and skills within a Windows based environment (MS Word, Excel, Outlook, PowerPoint, Access and SharePoint). Proficiency in data analytics and clinical dashboard applications. Ability to handle multiple priorities; strong organizational skills and the ability to meet deadlines in a fast-paced environment essential. Proven team member and ability to build stakeholder consensus in a complex environment. Ability to balance multiple demands and work on multiple projects simultaneously. Must possess exceptional judgment and ability to learn the needs of different components of the UR Medicine Enterprise. Work independently with minimal guidance. Demonstrates efficiency in prioritizing assignments, skill in proactively resolving problems and recommending and implementing continuous quality improvement and strong organizational skills. Strong oral and written communications skills are essential. Ability to multi-task required. Demonstrates accurate, grammatically correct documentation in all correspondence.
