Assist the health care providers with patient care in a specialty (Adolescent) interdisciplinary outpatient and inpatient practice. This includes direct patient care, documentation, and phone triage and clinically related administrative functions. Specific Responsibilities: Provide direct and indirect patient care services including but not limited to: Outpatient services: Obtain vital signs, perform orthostatic and assist with gynecologic procedures. Review and update electronic medical record including medication reconciliation. Monitor and assist with managing patient flow during regular clinic hours. Triage phone calls from patients, families and providers and consult with health care providers as necessary. Monitor and manage weekly clinical schedule and adjust accordingly. Monitor clinical utilization and track patient satisfaction. Provide patient/family/provider education as indicated. Assist providers with care coordination as indicated. Review all incoming clinical information to assess for appropriateness of referrals. In Patient services: Assists inpatient team with discharge coordination, engage in therapeutic meals for eating disordered patients as indicated and attend weekly inpatient eating disorder rounds. Identify patients eligible for eating disorder study and refer to study coordinator.
Requires a Master's Degree in Nursing or related field and an active New York State license in good standing. Applicants will have 5 years experience as an RN with one to three years of leadership experience; or equivalent combination of education and experience. The Nurse Practitioner influences the quality and delivery of care in various roles including consultant, researcher, mentor, and practitioner. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
Completion of an AMA approved Physician Assistant program, licensure in good standing, and certified as a Registered Physician Assistant. Demonstrated clinical competence, superior verbal and written communication skills, interpersonal effectiveness and customer relation skills, computer competence and must have an active desire to learn. This role may be unit, program, or service based and is involved in continuous quality improvement initiatives.
